Specialized Case Management

Working collaboratively with and through referrals from our Outreach teams and other agencies, our Case Managers/Specialists are able to extend more robust wrap around supports to individuals looking to accomplish health or social goals including housing, recovery, and health. They can help with paperwork, ID replacement, Income support, AISH applications, court processes and may be able to help with transport or appointment accompainment. Our Specialists have a thorough knowledge of service pathways in Central Alberta and provide a compassionate, trauma-informed and non-judgemental presence. Our Women's Specialist is a Licenced Practical Nurse and is focused on supporting vulnerable women, especially those at risk of or who are currently pregnant or postpartum. Our Referral Specialist can support individuals living with HIV or other STBBI's. Call our main office and ask for Raesi (Women's Specialist) or Ben (Referral Specialist).
